2. Fill out and complete all
sections of the form. Once complete, you can submit your order to
us and one of our Associates will contact you to complete your order.
Please make sure that if there are any pictures, logos or artwork, you
submit them in the highest quality. Additional information can be
submitted by using any of the following methods:

To Fax any sketches, artwork or
logos, please send to (973) 664-0003. Please note, any artwork or
logos will be reproduced to the best of our ability if received via fax.
It is recommended that if custom artwork & logos are to be included,
those digital files should be send to us at
orders@badge-printing.com.
Regular mail - If you chose, you may
send the complete package including the order form, pictures (actual
photos or media containing digital photos), logos and desired sketches
(if any) to:

3. Once we have validated your
information, we will create a "proof" that will be sent to you for your
approval. Before the proof can be created, the one-time setup fee
of $50.00 must be paid. We will then furnish the proof to you for
review and approval.
